General comments

Bij pO2 < 70 mm Hg of 8-9 kPa toevoegen prednison:

dag 1-5 2 dd 40 mg;
dag 6-10 2 dd 20 mg;
dag 11-14 1 dd 20 mg

Als een patiënt immunosuppressieve therapie gebruikt, maar op basis van onderstaande adviezen niet in aanmerking komt voor PCP profylaxe (bijvoorbeeld bij langdurig gebruik van prednisolon in doseringen <15 mg per dag, gebruik van een traditionele of biological DMARD, etc) of als er gekozen wordt geen profylaxe te geven, wees dan zeer alert op de mogelijkheid van het ontwikkelen van PCP en doe hier laagdrempelig aanvullend onderzoek naar.

     

    Indicatie

    Duur van profylaxe

    HIV-infectie:

    Als CD-4 lymfocyten < 0.2 * 10^9/l [3]

    Profylaxe geïndiceerd totdat CD-4 lymfocyten > 0.2 * 10^9/l gedurende 3 maanden

     

     

    Na solide orgaantransplantatie

    Na niertransplantatie

    Gedurende in ieder geval 6 maanden na niertransplantatie

     

     

    (Behandeling van) hematologische ziekten:

    Na allogene stamceltransplantatie [4]

    Profylaxe geïndiceerd tot minimaal 1 jaar na allogene stamceltransplantatie. Langere profylaxe kan nodig zijn bij persisterend gebruik van immunosuppressieve therapie na 1 jaar

    Na CAR-T cel behandeling

    Profylaxe geïndiceerd tot in ieder geval 3 maanden na CAR-T cel behandeling, mogelijk langer bij persisterende T-cel lymfocytopenie

    Bij patiënten met acute lymfatische leukemie (ALL) [4]

    Profylaxe geïndiceerd tot een maand na de maintenance fase van de behandeling

    Behandeling met purine analoga* (fludarabine, cladribine (2-CDA), clofarabine en bendamustine) [4]

    Profylaxe geïndiceerd tot

    • 6 maanden na behandeling
    • of als CD-4 lymfocyten > 0.2 * 10^9/l.

    Behandeling met alemtuzumab of ATG [4]

    Profylaxe geïndiceerd tot

    • 1 jaar na behandeling (vanwege langdurige T-cel depletie),
    • of tot minimaal 2 maanden na de laatste gift als CD-4 lymfocyten > 0.2* 10^9/l

    Bij patiënten die langdurig met prednisolon/dexametason behandeld worden (≥20 mg prednisolon of equivalent per dag gedurende > 4 weken) in combinatie met chemotherapie en/of anti-CD-20 therapie. Voorbeelden van deze schema’s zijn: R/O-CHOP-14 (niet bij R/O-CHOP-21), escalated BEACOPP, en sommige behandelschema’s voor multipel myeloom (zie hiervoor het behandelprotocol) [4]

    Profylaxe geïndiceerd tot 1 maand na afbouwen corticosteroïden tot <15 mg prednisolon of equivalent per dag, of 1 maand na laatste gift R/O-CHOP14.

    Behandeling met idelalisib of andere phoshoinositide 3 kinasse (PI3K) remmers [5]

    Profylaxe geïndiceerd tot

    •  6 maanden na afronden van therapie
    • Of tot minimaal 2 maanden na de laatste gift als CD-4 lymfocyten >0.2* 10^9/l

     

     

    (Behandeling van) solide tumoren:

    Radiotherapie (voor hersentumoren) in combinatie met temazolamide  of langdurig gebruik van corticosteroïden [6]

    Profylaxe bij gebruik van temazolamide geïndiceerd totdat de lymfocytopenie hersteld is.

    Behandeling met prednisolon ≥20 mg of dexametason ≥ 4mg per dag gedurende meer dan 4 weken (voor bijvoorbeeld hersenmetastasen, skeletmetastasen of behandeling immuun gemedieerde bijwerkingen van checkpoint remmers) [6]

     

     

     

    Bij immunosuppressieve therapie voor chronische inflammatoire aandoeningen

    Remissie inductie behandeling van granulomatose met polyangiitis (GPA) / andere ANCA-geassocieerde vasculitiden (AAV) (geschat risico >6%) [7]

     

    Behandeling met corticosteroïden in combinatie met cyclofosfamide (i.v. of oraal) (geschat risico >6%)

    Profylaxe geïndiceerd tot 4 weken nadat corticosteroïden zijn afgebouwd tot <20 mg prednisolon of equivalent per dag. 

    Behandeling met hoge dosis corticosteroïden (≥20 mg prednisolon of equivalent per dag gedurende 4 weken) in combinatie met:

    • Baseline lymfocytopenie (< 1,2 * 10^9/l) of CD-4 lymfocytopenie (<0,3 * 10^9/l) (geschat risico 2-6%) [8]
    • Overige biological (rituximab, TNF-alfa remmers) en non-biological (met name methotrexaat, cellcept) immunosuppressieve therapie (geschat risico 2-6%) [8]
    • Een startdosering van prednisolon 60 mg of meer, of chronisch corticosteroïdgebruik na een methylprednisolon stootkuur (geschat risico 2-6%) [8]

    Profylaxe geïndiceerd tot 4 weken nadat corticosteroïden zijn afgebouwd tot <20 mg prednisolon of equivalent per dag. 

    Patiënten behandeld voor inflammatory bowel disease (IBD) met triple immunosuppressieve therapie (geschat risico 2%) [9,10]
