step down na initiële behandeling

General comments

fluconazol step down na initiële behandeling ≥5 dagen met echinocandine: bij klinische verbetering, hemodynamisch gestabiliseerde patiënten, drainage van alle geïnfecteerde foci, en aangetoonde fluconazolgevoeligheid. Niet bij C. glabrata, C. krusei, of bij patiënten met intravasculaire of prothese-geassocieerde candidiasis.

Source control (debridement en drainage van alle geïnfecteerde foci) is essentieel.

Behandelingsduur afhankelijk van chirurgische en radiologische resolutie van geïnfecteerde foci.

Caspofungin bij een gewicht >80kg: 1dd 70 mg iv.
